Partly-printed DS, signed “Jay Silverheels,” one page both sides, 6 x 8.75, January 31, 1964. A designation and application for membership form for the American Federation of Television and Radio Artists (AFTRA) agency, signed twice by Jay Silverheels, who also adds his address, social security number, telephone number, agent's name, and profession as "Actor." In fine condition. Accompanied by a carbon copy of the payment receipt.
