Uncommon passport-style participant's identification booklet issued to Polish coach Ludwik Kostera for the Tokyo 1964 Summer Olympics, 3 x 4.5, gilt-stamped on the front with the 'rising sun' emblem of the Games. Issued as "No. 15113," the booklet has Kostera's portrait and signature inside, and is filled out in type with his nationality, address, and profession. In fine condition, with blurring to the signature.
