Alan Shepard’s Apollo-era ‘Travel Memorandum’ form from the Manned Spacecraft Center in Houston, Texas, one page, 8 x 10.5, dated March 7, 1969, filled out in ballpoint by Shepard, who signs his surname in the upper ‘Name’ field. Shepard annotates two separate trips: a flight from Houston to Cocoa Beach, Florida, on January 27, 1969, and a return from Cocoa Beach to Houston on February 1, 1969, both taken via “Gov Air.” In the field below, Shepard writes: “Rental car – receipt attached – 32 personal miles, Gov’t meals - Lunch on 1/29 - 1/30 - 1/31.” He then ticks a few boxes in the lower section. Included is the referenced NASA receipt for “General Travel Authorization,” issued to “Alan B. Shepard, Jr.,” on January 27, 1969, with the purpose listed as “Apollo 9 activities,” as well as copies of Shepard’s “Travel Voucher” and “Schedule of Expenses and Amounts Claimed” for the above two trips.
