Passport measures 3.5 x 4.75 and is filled out in type on the description page with her name, birth date, birthplace, and gender, with Welch signing below in black ink, “Raquel Welch Curtis.” Affixed to the adjacent page is her gorgeous 2 x 2 passport photo, signed vertically in black ink, “Raquel Welch Curtis.” The passport includes several visa stamps for England, France, Italy, Mexico, Japan, Saint Vincent and the Grenadines, Sint Maarten, Jamaica, and Barbados, dating between 1979 and 1983. Stapled inside the passport is a “VISA Application Form to Enter Japan,” dated March 19, 1980, which lists Welch’s various personal information, notes that her purpose to travel to Japan as “Guest Presenter, Japanese Academy Awards,” features an affixed glossy photograph of the actress, and is signed at the bottom in a secretarial hand.
