Description

Thrice-signed DS, one page, 8.5 x 12, April 24, 1967. A State Athletic Commission of California physical examination report for a professional boxing license, the upper portion of which is filled out in blue ballpoint by Joe Frazier, who signs his name twice, lists his address, and then answers “No” to the following 23 health and military-related fields, including “How many knockouts has he received.” A physician has filled out the second portion, noting right and left eye vision as “20/20-1” and “20/25,” and confirming Frazier’s overall good health. Signed again at the conclusion by Frazier. In fine condition. Less than a year later, Frazier obtained his first boxing title—the vacant NYSAC heavyweight title—when he beat Buster Mathis via TKO at Madison Square Garden on March 4, 1968.
