Fred Haise's training-used official ring-bound NASA checklist entitled "Apollo 16, April 16 Launch, Revision A, CSM Launch Checklist," approximately 120 pages, 6 x 8, dated March 7, 1972, signed on the front cover in blue ballpoint, "My personal training copy—Fred Haise, Apollo 16 BU CDR." Prepared by the Manned Spacecraft Center's Systems Procedures Branch, the manual covers liftoff configuration, boost preparation, launch trajectory, launch aborts, boost, orbit insertion and orbital check-outs, TLI preps, booster (S-IV-B) separation, re-entry, and emergency procedures. In fine condition, with wear, creasing, and staining to the covers from use.
After evading disaster on Apollo 13, Haise remained in the astronaut rotation and served as the backup mission commander for Apollo 16; he was prospectively slated to command Apollo 19 prior to the mission's cancelation. After completing his Apollo 16 backup assignment, Haise moved to the Space Shuttle program and conducted the Space Shuttle Enterprise's Approach and Landing Tests in 1977.
