The form is signed and approved by two physicians and is signed at the conclusion in black ballpoint by Joe Frazier. In fine condition. An Athletic Commission receipt stamp dates to May 5, 1967, the day after Frazier improved to 16-0 with a unanimous decision win over George Johnson. He would go undefeated over the next five-and-a-half years, including a win over Muhammad Ali on March 8, 1971, before losing to George Foreman via TKO on January 22, 1973.
