Duplicate of a "Prescription Form for Medicinal Liquor," issued under the authority of the National Prohibition Act, one page, 5.5 x 4.5, listing the liquor as "Whiskey," and a prescription date of June 2, 1930. The lower section is signed and numbered by the dispenser, who filled the order the following day in Sacramento, California. Reverse bears instructions which note that the "Prescription is invalid after 3 days." In fine condition.
