DS, signed "Mathew Carey, twenty five copies at one third & nine months," one page, 8 x 9.75, no date. Subscription form for the publication of Ridgley's Body of Divinity, neatly signed at the conclusion by Carey. In very good condition, with several edge tears, and some creasing to the lower right corner. Accompanied by an engraved portrait bearing a facsimile signature.
