American actress (1880-1962) who is best known for her memorable role as Auntie Em in The Wizard of Oz. DS, signed “Clara Blandick,” one page, 8.5 x 11, June 29, 1936. Studio employee personnel record filled out in type and signed at the conclusion in fountain pen by Blandick. The document lists her name, Hollywood address, phone number, and physical characteristics. Items of note include "Date Born" ("Refuses to state"), and "Place of Birth" at "Sea"—she was in fact born aboard the ship Willard Mudgett, captained by her father, on June 4, 1876. Stamped at the top, "Received, June 29 1936, Accounting Dept." In fine condition, with trimming to the top, presumably done by the studio.
