Application for membership in the American Federation of Television and Radio Artists (AFTRA), signed twice at the conclusion by Williams, and filled out with his personal information. A blue carbon copy receipt is stapled at the top. In fine condition. Accompanied by a picture sleeve for the 45 RPM record 'Slow Down / Dizzy, Miss Lizzy.'
