The first side is headed “Alarm Codes” and features a list of 14 codes with definitions and actions; the back side, labeled “Apollo 15 Data” and headed “Mission Rules No-Go’s,” lists various options for potential failures during powered descent initiation, with ink and pencil notations in upper right, “LMS 2” and “102–04–15.” In fine condition.
