One of Forte’s official game-used (practice) orange Chicago Bears jerseys, with printed on numbers. Signed on the number on the front in blue felt tip, “Matt Forte, game used.” The Reebok label is sewn into the lower left tail, indicating a size of 44, and a label sewn into the collar which reads, “08-44.” A small North Shore University Health System patch is sewn to the upper left shoulder area. In fine condition, with signature slightly faded, but still completely legible, and a PSA/DNA authentication label affixed to tag. Accompanied by a certificate of authenticity from PSA/DNA.
