Form provides space for reporting gross earnings as a member of the Guild, and indicates Kubrick’s earnings at $12,000 for 1960 with him predicting “same??” for 1961. Signed at the conclusion in black ink by Kubrick. Intersecting folds, vertical strip of adhesive residue to left side, and other expected document wear, otherwise fine condition.
