McPhatter’s rare yellow AFTRA union P&W benefits card, one page both sides, 6 x 4, dated January 7, 1964. Pension and Welfare benefit card filled out in McPhatter’s hand in blue ballpoint including his full name, New York City address, birthdate, and Social Security number. Signed at the bottom “Clyde McPhatter.” A stamp across the front reads “Deceased.” On the reverse in another hand his mother is listed as beneficiary. Several office notations affecting portions of writing including the address being crossed out and written anew in another hand, and the “Deceased” stamp affecting first name of signature, otherwise fine condition.
