Costello’s rare off-white AFTRA union P&W benefits card, two pages stapled together, 6 x 4, no date. Pension and Welfare benefit card filled out in Costello’s hand in blue ballpoint including his full original name, California address, birthdate, Social Security number, and noting his wife as beneficiary “Mrs. Lou Costello.” Signed at the bottom with his full, given name “Louis F. Cristillo.” A handwritten note across the front of the card reads “Deceased 3/3/59.” On the second card, stapled to the reverse of the first, Costello pens his wife and children as his beneficiaries. In very good condition, with several office notations, with one affecting the first letter of last name of formal signature, the “Deceased” notation affecting several portions of Costello’s writing on the form, toning from tape remnants to top and bottom edge of both cards, and aforementioned stapling in two places keeping the cards attached. A rare piece of Hollywood history with the very uncommon use of Costello’s given name—Cristillo. Pre-certified PSA/DNA and RR Auction COA.
