Signed in the middle by Lamarr, and signed again at the bottom by Lamarr in an area labeled “Please sign your name again in space below for reproduction of autograph.” In very good condition, with central vertical fold, one through a single letter of signature, scattered toning and soiling, punch holes to left edge, staple to top edge, and ink notation to bottom.
