Partly printed DS, signed “Th: Jefferson,” one page on vellum, 12.25 x 7.25, April 2, 1804. An appointment for [first name illegible] Read in the Navy of the United States. In good to very good condition, with several filled-in words faint and only partly legible, small hole to top margin (evidently original to manufacture of sheet), moderate soiling and light toning, and a few light creases and wrinkles touching signature.
