The Personnel Record details usual information, i.e. address, date of birth, social security number, SAG and Equity membership, etc. The 'In case of accident/relationship' section of the card appears to have been filled out and then vigorously written over. Full and quite large blue ink signatures on both sides of document. In fine condition with usual office wear, a file hole through part of her signature on the reverse. RRAuction COA.
