Seinfeld fills several of the questions in ballpoint, incorporating his signature, “Jerry Seinfeld,” into the first field marked as ‘Name Presently Used.’ Other answered fields include: Given Name: “Same”; Date of Birth: “4/29/54”; Local Neighborhood: “Borough Park”; Cross Streets-Residence: “52nd St.”; First Big Break: “1st Tonight Show 1981, 1st Letterman 1982”; Your Feelings About Growing Up in Brooklyn: “Only lived there 1 year. My memories are all of rattles and spinning mobiles.” Seinfeld appears to have mistaken the field ‘Hospital Born In’ for an education question, as his response reads: “Brooklyn College.” The lower section bears an affixed ballpoint signature, “Seinfeld, 129 W 81st, NYC 10024,” clipped from a mailing envelope; the address is amusing given that 129 West 81st Street was the location of his Manhattan apartment in the show Seinfeld. In very good to fine condition, with slightly irregular toning, and old clear tape along the back of the top edge. Accompanied by a full letter of authenticity from JSA.
