Surgeon and educator (1753–1815) who served as a Continental Army surgeon during the American Revolutionary War, founded Harvard Medical School, and was the younger brother of Dr. Joseph Warren. Scarce partly-printed DS, one page, 13 x 9, June 9, 1791. Massachusetts Humane Society membership certificate issued to Caleb Hayward, prominently signed at the conclusion in ink by John Warren as the organization's president, and countersigned by Edward Gray as secretary. In very good to fine condition, with scattered light foxing, and toning and chipping to the edges.
