American actress, socialite, fashion model, and heiress (1943-1971) who is best known for being one of Andy Warhol's superstars. Partly-printed DS, signed "Edith M. Sedgwick," one page, both sides, 6 x 4, September 3, 1965. Yellow AFTRA Pension and Welfare benefits card filled out by Sedgwick in black ink, who prints both her full legal name and her professional name, "Edith Minturn Sedgwick" and "Edie Sedgwick," her New York City address at "16 East 63rd St.," her birthdate, and her Social Security number. Signed at the conclusion in neat fountain pen by Sedgwick. On the reverse, she lists her mother, "Mrs. F. M. Sedgwick," as the recipient of her death benefits. Archivally and tastefully double-matted and framed with an engraved plaque and two photos to an overall size of 32.25 x 17.25. In fine condition. Sedgwick remains exceedingly rare across all signed formats, with this multi-signed AFTRA document representing the only example we have ever offered; additionally, the year this card was issued, 1965, dates to Sedgwick's emergence as a Warhol superstar and fashion influencer.
