The following page is filled out in type with his full name, birth date, height, birthplace, and hair and eye color, with Hashian signing below, “John Thomas Hashian.” Affixed to the adjacent page is his 3 x 3 passport photo, signed along the edge in ballpoint, "John Thomas Hashian."
