The accompanying document represents the actual "Authorization" page for the Apollo 17 Lunar Surface Checklist. It was removed from the front of the flown checklist, within which it spent three remarkable days on the surface of the Moon. The page measures 5.5 by 8 inches and is dated November 6, 1972, and also bears Cernan's signed flight-certification stamp. Removably-affixed to a deluxe 13 by 19 inch presentation mount bearing a portrait of Cernan holding the checklist from which this page originated.
