DS, signed “Archie Lee Moore,” one page, 8.5 x 13, February 21, 1959. State Athletic Commission of California physical examination report for Moore’s license application as a referee. Detailed report, filled in in another hand, states Moore has spent 23 years as a boxer, his last knockout coming in 1956, his heart rate and reports he had an appendectomy. Signed at the bottom by Moore, as well as the physician. In fine condition, with punch holes to top edge, a few creases, and a stamp to lower left. Pre-certified Steve Grad/PSA/DNA and RRAuction COA.
